Once the enrollment process is complete you will have the chance to review and confirm your entries before submitting your enrollment information. An email confirmation will be sent to the email address you provide during the enrollment process. In addition, a separate confirmation of your enrollment will be mailed to the address you provide. This confirmation will include a default PIN number you will need for accessing your account online. Write down this PIN number and keep it in a safe place for future use.

The Salary Reduction Agreement (SRA) is utilized to establish, change, or cancel salary reductions withheld from your paycheck and contributed to your 403(b) Plan on your behalf. You may change the amount that you are contributing at any time with this form. Upon completion, fax or mail a copy of the form to Capital Street Financial Services, Inc. Please note that this form is not valid unless all applicable sections are completed and you have signed the form. Allow up to two pay periods for the changes to be reflected on your pay-stub.
