Once the enrollment process is complete, you can review and confirm your entries before submitting your enrollment information. An email confirmation will be sent to the email address you provide during the enrollment process. In addition, a separate confirmation of your enrollment will be mailed to the address you provide. This confirmation will include a default PIN you will need to access your account online. Please write down this PIN and keep it safe for future use.

The Salary Reduction Agreement (SRA) establishes, changes, or cancels your Voya 403(b) Plan contributions. You may change the amount you contribute at any time with this form. After you complete it, fax or mail a copy of the form to Capital Street Financial Services, Inc. Allow up to two pay periods for the changes to be reflected on your pay stub.
