Once the enrollment process is complete, you can review and confirm your entries before submitting your enrollment information. The Salary Reduction Agreement (SRA) establishes, changes, or cancels your Voya 403(b) Plan contributions. You may change the amount you contribute at any time with this form. After you complete it, fax or mail a copy of the form to Capital Street Financial Services, Inc. Allow up to two pay periods for the changes to be reflected on your pay stub.
